As a Registered Psychotherapist in Ontario, this policy should be reviewed against PHIPA and CRPO's confidentiality standards before it's treated as final for real clients — this is a strong starting draft, not a substitute for that review.

Health Information, Confidentiality & Legal Compliance

This practice operates in accordance with:

  • The Personal Health Information Protection Act (PHIPA) of Ontario, which governs how health information (such as clinical records and session notes) is collected, used, and disclosed
  • The College of Registered Psychotherapists of Ontario (CRPO) Code of Ethics and Professional Practice Standards
  • The Canadian Counselling and Psychotherapy Association (CCPA) Code of Ethics
  • The Personal Information Protection and Electronic Documents Act (PIPEDA), which may apply to personal information collected through this website that falls outside of health information covered by PHIPA — such as general contact form submissions from a non-client, or website analytics

Limits to confidentiality

Confidentiality is a cornerstone of this practice, but it is not absolute. Information may be disclosed without consent where required by law — including where there is a risk of serious harm to yourself or others, a court order or subpoena, or a legal duty to report suspected abuse of a child. These limits are also explained verbally before therapy begins.

Security & Data Retention

Appropriate technical and organizational measures are used to safeguard your information, including secure cloud storage, access controls on sensitive files, and restricted access to client information.

Form submissions and clinical records are kept only as long as necessary to fulfill the purpose they were collected for, or as required by CRPO record-keeping standards and applicable law.

Third-Party Services

To support scheduling and communication, this practice uses Jane App for appointment booking and scheduling.

Any third-party service used has its own privacy policy governing the information it handles.

Your Rights & Alternative Ways to Reach Us

You have the right to:

  • Know what information is held about you
  • Request correction of inaccurate information
  • Withdraw consent at any time, subject to legal or clinical exceptions
  • Request deletion of a form submission, where applicable

Using the online forms on this site is entirely optional — you're never required to submit one to access therapy or reach us. If you'd rather not use a form, you can email flipsmith@dosetherapy.co directly, or book through Jane App.
